While you may use some of the Services without registering, certain Services do require that you register with Mango Health for full functionality. Additionally, registering enables you to access your account information on multiple devices and enables you to recover your information in the event you change mobile phones or reinstall the application on your mobile device. If you choose to register or update an existing account with Mango Health or access certain Services, you may be required to provide certain personal information, such as your name, address, telephone number, gender, email address and date of birth, and a username and password to access your account. You are responsible for ensuring the accuracy of the personal information that you submit to Mango Health.
If you download or use the Mango Health app in connection with a program offered by an employer, healthcare provider, pharmacy benefit manager, or other healthcare professional in collaboration with us (collectively, "Benefit or Healthcare Provider"), you may be required to register with such Benefit, pharmaceutical manufacturer orHealthcare Provider in connection with such program and provide certain Personal Information to such provider. We are not responsible for the use of any Personal Information that you may decide to provide to any Benefit, pharmaceutical manufacturer or Healthcare Provider in connection with the use of the Services.
If you choose not to register, we will assign a unique identifier ("UID") to your device in order to send you medication reminders, notices or other messages, and to provide appropriate content each time you use the Mango Health app. If you download the Mango Health app to your mobile device, use it for some time, delete it and then reinstall it, we would create a new UID for you at the time of the reinstall. This means that we would not associate the user with the previous UID and would not show your previous history in Mango Health. If you are an unregistered user who has disabled push notifications, you acknowledge and agree we may not be able to contact youwith information or updates about the Services.
"Personal Information" means any information that may be used, either alone or in combination with other information, to personally identify an individual, including, but not limited to, a first and last name, a personal profile, an email address or other contact information. The categories of Personal Information we collect depend on the Services and whether you are a customer, user, applicant or visitor, all subject to applicable law.
Health Information. We also collect certain health information (including medications or health status).

In addition, if you access our Services through a third-party application, such as an app store, a third-party login service, or a social networking site, we may collect information about you from that third-party application that you have made public via your privacy settings. Benefit/Healthcare Provider Activation Codes. If you download or use the Mango Health app in connection with a program offered by a pharmaceutical manufacturer or benefits or healthcare provider, enter an activation code provided to you by the third party or otherwise authenticate to a benefit or healthcare provider in the app, we may provide Personal Information about you to the third party, including data related to your use of, and interaction with, the Services. The third party may be able to associate the Personal Information we provide with you.
Gift Program. In connection with your participation in the Mango Gift Program, we may share your Personal Information with merchants that we have a contractual relationship with, for the purpose of redeeming and fulfilling your reward or notifying you that you have been selected as a winner of a gift (such merchants are obligated to use your Personal Information only to redeem and fulfill your reward and are obligated not to disclose it or use it for any other purposes).
Service Providers. We may share any Personal Information we collect about you with our third- party service providers. The categories of service providers to whom we entrust Personal Information include: IT and related services; information and services; payment processors; customer service providers; and vendors to support the provision of the Services.
Business Partners. We may provide Personal Information to business partners with whom we jointly offer products or services. In such cases, our business partner's name will appear along with ours.
Affiliates. We may share Personal Information with our affiliated companies for their use in developing associated products and services or for analytics associated with the services they provide
Advertising Partners.
